Role Description
The Patient Care Manager will lead care coordination efforts, support patient engagement, and ensure the delivery of high-quality, patient-centered services across virtual care platforms. This role serves as a key liaison between patients, healthcare providers, and internal teams, ensuring seamless communication and effective care management. Key Responsibilities
• Oversee daily patient care coordination and remote support services
• Supervise, mentor, and support patient care coordinators and support staff
• Ensure timely patient follow-ups, care updates, and service assistance
• Monitor patient cases, escalations, and service quality standards
• Collaborate with healthcare providers to align care plans and patient needs
• Maintain accurate patient records and ensure HIPAA compliance
• Analyze patient care metrics and implement process improvements
• Train staff on patient communication best practices and care protocols
• Address complex patient concerns with professionalism and empathy
Qualifications
• Bachelor’s degree in Healthcare Administration, Nursing, Public Health, or a related field
• Minimum of 1 year of experience in patient care coordination, healthcare operations, or case management
• Prior leadership or team management experience
• Strong understanding of healthcare workflows and patient service standards
• Excellent communication, organizational, and problem-solving skills
• Experience with EMR/EHR systems and remote care tools
• Ability to manage multiple patient cases while maintaining high-quality service
Preferred Qualifications
• Nursing background (RN/LPN) or clinical experience
