Description Shared Living Providers are contracted through 3Rivers to support an individual with intellectual disabilities or autism according to their person-centered plan.  These individuals live in the home with the Shared Living Provider. The Shared Living Provider works with the individual on all aspects of daily living, which may include administering medications, transportation, coordinating with other care providers, support medical appointments, improving safety skills, interpersonal skills development, nutritional support, behavioral interventions, and personal care.  Shared Living Providers are members of an individuals multi-disciplinary team. At 3Rivers, we believe in the concept of co-managing with the individual which means support is viewed as not just assisting but imparting skills needed for independence.  Shared Living Providers balance health and safety with the dignity of informed risk and individual choice, as well as balancing a healthy, positive relationship built on trust and respect. 3Rivers provides the following supports to Contracted Shared Living Providers: - Web-based electronic documentation - 24-hour on-call support - Competitive daily rates (tax free!) with weekly direct deposit - Training - Home safety inspections - Provider recruitment and background checks - Monthly contacts - Participation in all team meetings - Coordination with other providers - Consultation on matters of compliance Requirements - Must be at least 18 years old. - Must have a high school diploma or GED. - Valid State of Maine Driver's License. Summary For many individuals with intellectual disabilities or autism, traditional group home settings are not appropriate.  This could be due to personal choice or a level of support issue.  Independent living may also not be an options for similar reasons. To better serve the spectrum of need while maximizing opportunity for personal choice and growth, 3Rivers offers a shared living option, sometimes referred to as "adult foster care". This option places the individual served into a home with a family or supportive roommate within the community of their choosing.  Shared Living Providers are contracted through 3Rivers to support the individual according to their service plan.
