Job Description:
We are seeking an experienced LVN or higher-licensed nurse to support an established Remote Patient Monitoring (RPM) program. This is a 100% remote, work-from-home position for a USA-based nurse who can start immediately.
Direct RPM experience is required.
This role is not suitable for entry-level nurses or candidates without hands-on RPM experience.
Primary Responsibility (Initial Focus):
-Onboard new RPM patients, including outbound patient outreach by phone
-Educate patients on the RPM program and expectations
-Guide patients through device setup and activation (BP cuffs, scales, pulse oximeters, glucose monitors, etc.)
-Ensure patients are successfully transmitting readings and feel confident using their devices
-Ongoing Responsibilities:
-Review and monitor RPM patient data (blood pressure, weight, glucose, pulse oximetry, etc.)
-Conduct patient follow-ups and ongoing engagement via phone or video
-Identify and escalate clinical concerns according to established protocols
-Document patient interactions accurately and compliantly
-Collaborate with physicians and clinical care teams
-Support long-term patient adherence to monitoring programs
Requirements (Must Have):
-Active LVN license or higher (RN preferred)
-Licensed and residing in the United States
-Proven Remote Patient Monitoring (RPM) is ideal but not necessarily required
-Experience with patient onboarding and phone-based outreach
-Strong clinical judgment and communication skills
-Comfortable using EHRs, RPM platforms, and telehealth tools
-Reliable internet connection and ability to work remotely
-Available to start immediately
Nice to Have:
-Experience with Chronic Care Management (CCM)
-Familiarity with CMS RPM guidelines and workflows
-Prior telehealth or virtual care experience
