Summary The Remote Clinical Manager is a remote leadership role that directs, coordinates, and evaluates the delivery of home health services to a team of clinicians ensuring compliance and consistency with agency policy and State and Federal home health regulations. This position requires an active RN license in the states required to manage care plans, compliance, and staff training.  The Remote Clinical Manager will fill in as needed across the enterprise to ensure continuity of care.   Primary Responsibilities: - Oversee all patient care services and personnel for a clinical team to include coordinating referrals and patient care, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the patient’s individualized plan of care. - Implements and supervises all patient care activities to ensure the delivery of safe, economical, and efficient patient care, which adheres to relevant standards of accepted nursing and medical practice, to include federal and state regulations, as well as to ensure compliance with payor source criteria. - Assists in policy review and revisions as requested to establish and support the highest possible quality of patient care, cost controls, quality assurance, and staff performance. - Acts as a liaison between staff, patients, the medical community, and upper management to ensure information is shared appropriately among the agency’s medical team. - Participates in the QAPI plans and processes, including the review of Plans of Care. Ensures follow up on any required action plans. - Reviews weekly dashboard report to ensure the organization makes regular progress against its strategic goals and addresses issues raised through metrics. - Leads weekly case conference and addresses recert / discharge decisions based on case conference findings in the absence of current FT clinical manager/branch director. - Supervises the scheduling process from referral through discharge to ensure that clients receive assessment visits and that these visits are scheduled and performed timely per agency policy. Ensure that referrals are appropriate for home health. - Serve as a liaison for in-field nurses and clinicians. Help troubleshoot and provide support for clinicians. - Where able, may be asked to participate in the backup supervisor on-call rotation. - Completes all other duties as assigned and consistent with the nature of this work.   Qualifications: - Previous experience in a clinical manager role. - In-depth understanding of Medicare Conditions of Participation and payment mechanisms for home health. - Strong organizational, planning, problem solving, decision-making, and leadership skills required. - Strong interpersonal skills. Ability to communicate effectively in writing and verbally with people who have varied background. - Ability to direct and manage a multi-disciplinary team of home health care clinicians, both professional and para-professional.   Requirements: - Registered Nurse with two (2) years in community health / home health program. - Experience with HCHB required. - Bachelor’s degree in related field preferred. - License and current registration to practice in related discipline in the state where job is located. Must be licensed, or willing to become licensed in the following states: MO, IL, OH, MI, and KS. - Computer skills and ability to use word processing, databases, spreadsheet, applications, etc. preferred.
