Job Overview: Ebenezer is looking for a Registered Nurse (RN) Triage to join our corporate team! This position provides Registered Nurse virtual support to Assisted Living site staff during off-hours when there is not a nurse in the building. This RN triage schedule includes; - 40 hours every two weeks - Evening/Overnight (NOC) shift; 5:00 PM – 8:00 AM - Includes every other weekend & holiday rotation Responsibilities:  - Provides nursing coverage in a triage nurse on-call capacity. - Ensures support/response to client’s health related situations for staff, client, and family 24 hours a day through phone triage. - Ensures appropriate communications between staff, family, and providers to coordinate client health needs as assessed in the triage on-call role. - Ensures appropriate triage phone assessment, direction, and action for clients medical situations based on information provided by caller. - Directs on-call visit nurse to make onsite visits as determined by triage assessment. - Provides nursing triage on-call coverage for single or multiple assisted living buildings as assigned. - Follows triage nurse on-call assignment expectations - Performs other duties as assigned Required Qualifications: - Associates Degree in Nursing or Vocational/Technical Training - 1 year One to Three Years - Proficient in Microsoft Windows 7 - Registered Nurse RN Licensure/Registered with the MN Board of Nursing - Registered Nurse Wisconsin RN Licensure/Registered with the WI Board of Nursing (required within 90-days of hire) within 90 Days Preferred Qualifications: - Bachelor of Science Nursing - 3 years Three to Five Years - Proficient in Eldermark Computer Documentation Software - Prior supervisory experience - Prior experience in home care or assisted living environment - Basic Life Support (American Heart Assoc or Red Cross) Current Basic Life Support (BLS) for Health Care Provider from American Heart Association (AHA) or American Red Cross; accepted certifications as follows -AHA: BLS for Healthcare Providers (CPR & AED) Program or BLS Provider - BLS Instructor.
