This role requires independence, strong clinical skills, and the ability to adapt to diverse home environments while adhering to Medicare and ICAHO guidelines.   Key Responsibilities - Implement treatment plans as directed by the supervising Physical Therapist, ensuring adherence to frequency and duration specifications. - Document patient progress, strength, ROM, balance, and functional mobility using objective measures. - Communicate effectively with patients, caregivers, physicians, and the rehab team to coordinate care. - Conduct discharge planning and make referrals for ongoing services as needed. - Provide patient/caregiver education and address safety needs. - Maintain accurate and timely documentation in compliance with agency, state, and federal regulations. - Participate in continuing education, in-services, and departmental initiatives.   Qualifications - Education: A.A. Degree from an approved Physical Therapist Assistant program. - Licensure/Certification: - Current Maryland Physical Therapist Assistant License. - Certification in Adult and Pediatric CPR. - Valid Maryland Driver’s License, auto insurance, and reliable transportation. - Experience: 1+ year in clinical care (acute, subacute, or home health preferred). - Skills: - Strong interpersonal and communication skills. - Ability to work independently and adapt to varied home environments. - Excellent time management and organizational skills. - Knowledge of infection control and emergency preparedness.   Salary Range: Minimum $26.58/hour - Maximum $43.89/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
