**Job Summary**
We're seeking an experienced Case Management Analyst to join our team on a part-time basis. As a Case Management Analyst, you'll be responsible for assessing the needs of individuals and developing personalized care plans to help them achieve their health and wellness goals. You'll work closely with a team of healthcare professionals, social workers, and other stakeholders to ensure that each individual receives the highest level of care and support.
**Key Responsibilities**
* Conduct comprehensive assessments of individuals' needs and develop personalized care plans to address their health and wellness goals
* Identify high-risk factors and service needs that may impact individual outcomes and collaborate with healthcare professionals to develop strategies for addressing these issues
* Facilitate and implement assigned care plan activities and monitor progress

**Essential Qualifications**
* Bachelor's degree in a related field (e.g., social work, psychology, public health)
* 2+ years of experience in case management or a related field
* Strong knowledge of case management principles and practices
* Excellent communication and interpersonal skills

* Strong analytical and problem-solving skills
* Proficiency in Microsoft Office applications (Word, Excel, PowerPoint)
