What We’re Looking For
Trillium Health Resources has a career opening for a Director of Health Outcomes to join our team! The Director of Health Outcomes is responsible for advancing enterprise-level clinical initiatives that drive organizational alignment, quality performance, and integrated care transformation. This role supports Trillium’s strategic and quality plans by building the infrastructure, tools, and cross-functional systems necessary to deliver scalable, data-informed care. Positioned at the convergence of care management, quality, and population health, the Director administers the operationalization of the Enterprise Quality Driven Care Plan (QDC), supports performance reporting, and fosters innovation through enterprise collaboration. This is a remote based position, the ability to travel onsite as needed for Trillium business is required.
On a typical day, you might:
- Shape the strategic execution of Trillium’s clinical quality and performance agenda, aligning enterprise initiatives with regulatory standards, internal priorities, and Medicaid transformation goals.
- Serve as sponsor of the Quality Driven Care Plan (QDC), overseeing the design and implementation of system-wide tools, data frameworks, and governance models that enable organizational visibility, accountability, and strategic alignment.
- Lead the development of PHOENIX, a centralized program index that maps clinical initiatives to HEDIS metrics, departmental objectives, and state benchmarks—creating enterprise-wide transparency and tracking capabilities.
- Guide the Care Management data engineering team that builds dashboards and reporting tools supporting operational and clinical outcomes.
- Drive alignment and integration among Care Management, Quality Management, and other divisions to foster shared accountability and unified action in support of accreditation, performance, and innovation.

Qualifications
Required:
- Bachelor’s degree in Public Health.
- Minimum of three (3) years of progressive experience in healthcare strategy, clinical transformation, or system-level quality improvement.
- Must have a valid driver’s license.
- Must reside within United States.
Preferred:
- Master’s degree preferred; Healthcare Administration, Public Health, Social Work, Business Administration, IT, Human Services, or a related field.
- Supervision of two (2) or more employees.
- Experience in leading and managing complex, moderate to large-scale implementation projects within a healthcare or business setting.
- Knowledge of integrated care models, principles, and best practices.
- Understanding of change management methodologies and the ability to drive organizational transformation.
- Certificates or industry certifications in Project Management, Program Development, Change Management, Business, Strategy, Data Analytics, or Leadership, or equivalent certifications.
